<p> <table cellspacing="0" cellpadding="0" width="100%"><tbody><tr><td><p> </p></td></tr></tbody></table><strong>خلاصه </strong><strong></strong></p><p> <strong> سابقه و هدف </strong>: یکی از عوامل دخیل در موفقیت انتقال جنین حذف موکوس دهانه رحم است. بررسیها نشان میدهد میزان حاملگی در زنان با کشت مثبت ترشحات سرویکس بسیار کمتر از میزان حاملگی در زنان با موکوس عاری از باکتری میباشد، همچنین وجود خون یا موکوس آلوده در نوک کاتتر انتقال جنین، منجر به آسیب دیدگی جنین و جایگزینی نامناسب در رحم میگردد. بنابراین هدف از این مطالعه بررسی تأثیر حذف موکوس بر نتیجه انتقال جنین بود . </p><p> <strong> </strong><strong>مواد و روش </strong><strong> </strong><strong>ها: </strong>در این کارآزمایی بالینی از بین 340 زن که جهت انجام IVF یا ICSI به مرکز تحقیقاتی درمانی ناباروری یزد و بیمارستان مادر مراجعه کردند، برای 170 زن (گروه A یا گروه مورد) تخلیه موکوس سرویکس به وسیله سرنگ انسولین انجام شد و اینها از نظر نتیجه حاملگی با 170 زن (گروه B یا گروه شاهد) که موکوس سرویکس آنها فقط ب ه وسیله سواب پنبهای پاک شده بود، مقایسه شدند. تحریک تخمدان در دو گروه به روش پروتکل دراز مدت بوده است. نتیجه حاملگی به صورت تیتراژ مثبت ß -HCG ، 14 روز پس از انتقال جنین در دو گروه دادهها جمع آوری گردید ند . سپس اطلاعات با استفاده از آزمونهای آماری مجذور کای و آزمون t تجزیه و تحلیل شد ند . </p><p> <strong> یافتهها: </strong>دو گروه از نظر میانگین سن زن و مرد و طول مدت و علت نازایی تفاوت معنی داری نداشتند میانگین سن مرد در گروه A ، 2/5 ± 26/35 سال و در گروه B ، 4/5 ± 13/35 سال و میانگین سن زن نیز در گروه A ، 3/5 ± 8/29 سال و در گروه B ، 4/5 ± 2/30 سال بو د. میانگین مدت نازایی هر دو گروه 6/4 ± 6/8 سال بود. میانگین تعداد فولیکول در گروه A ، 2/11 و در گروه B ، 7/10، میانگین تعداد اووسیت در گروه A ، 1/7 و در گروه B ، 7/6 و میانگین تعداد جنین در هر دو گروه 2/3 بود که در هیچ یک از موارد فوق تفاوت معنی داری بین دو گروه وجود نداشت. از نظر علت نازایی و روش انجام کار آزمایشگاهی نیز دو گروه تفاوت معنیدار ی نداشتند. اگرچه در گروه A میزان حاملگی بیش از گروه B بدست آمد (9/26 درصد در مقابل 2/19 درصد) اما از لحاظ آماری معنیدار نبود. </p><p> <strong> نتیجه </strong><strong> </strong><strong>گیری: </strong>در این مطالعه تخلیه کامل موکوس دهانه رحم با استفاده از سرنگ انسولین با افزایش میزان حاملگی همراه بود ، که میتواند از لحاظ بالینی دارای ارزش باشد . این موضوع به وسیله مطالعات دیگر هم مورد تایید قرار گرفته است. </p><p> <strong> واژههای کلیدی: </strong>تلقیح مصنوعی، موکوس سرویکس، انتقال جنین <strong></strong></p><p> </p>

Cervical mucus aspiration is the most important factor in successful embryo transfer. Based on reports pregnancy rate in woman with positive culture of cervical mucus is lower than women with negative culture. Mucus plague on the catheter tip can cause improper embryo implantation. This study evaluated the effect of cervical mucus aspiration on embryo transfer results. </p><p> <strong> Materials and Methods </strong>: A Randomized Clinical trial study was performed on 340 women (in two groups) by cycles of ART (Assisted reproductive technology) such as IVF (In vitro Fertilization) or ICSI (Intra cytoplasmic sperm injection) referred to the Research and Clinical Center of Infertility and Madar Hospital of Yazd. Male's and female's age, duration and cause of infertility were mached in two groups. The long time protocol was used for induction of the ovaries. In 170 patients, cervical mucus aspiration was done with insulin syringe (A or case group). In the control group or group B (170 women), cervical mucus was aspirated only with cotton swabe. The patients were followed up by ß-HCG serum level measurement on the day 14 after embryo transfer. The data were analyzed by Chi-square and t-test . </p><p> <strong> Results: </strong>Mean of men's age in group A was 35.26±5.2 years and in group B was 35.13±5.4 years. Mean of women's age in group A was 29.8±5.3 years and in group B was 30.2±5.4 years. Mean of infertility duration was 8.6±4.6 years in both groups. Mean of follicle number, oocytes number, embryos number, cause of infertility and IVF/ICSI method in both groups were similar and there was no significant difference. Pregnancy rate in group A, was more than group B, (26.9% Vs 19.2%), but was not statistically significant.<strong></strong> </p><p> <strong> Conclusion </strong><strong>: </strong>Cervical mucus aspiration with insulin syringe before embryo transfer can increase the pregnancy rate.
