مقایسه داروهای اندانسترون، متوکلوپرامید و دگزامتازون در پیشگیری از تهوع و استفراغ در کودکان تحت عمل جراحی استرابیسم
Comparison of the Antiemetic Effects of Ondansetron, Metoclopramide and Dexamethasone in Children Undertaken Strabismus Surgery

<p align="right"> <strong> زمینه و هدف: </strong>تهوع و استفراغ از عوارض مهم بعد از اعمال جراحی است که برای بیمار ناخوشایند بوده و سبب تأخیر در ترخیص و بروز عوارض دیگر میشود. رویداد این عارضه بعد از جراحی استرابیسم زیاد است. روشهای دارویی و غیر دارویی متعددی برای پیشگیری از بروز این عارضه به کار رفته است. هدف این مطالعه مقایسه اثرات سه داروی اندانسترون، متوکلوپرامید و دگزامتازون در پیشگیری و یا کاهش تهوع و استفراغ به دنبال جراحی استرابیسم در کودکان بوده است. </p><p align="right"> <strong> مواد و روشها: </strong>در این مطالعه کارآزمایی بالینی دوسویه کور، 111 کودک و نوجوان 2 تا 15 ساله، کلاس ASA <br>( Physical Status Class ) I و II کاندید جراحی الکتیو استرابیسم به صورت تصادفی در 3 گروه اندانسترون، متوکلوپرامید و دگزامتازون قرار گرفتند. دگزامتازون در شروع عمل و به دنبال القاء بیهوشی و اندانسترون و متوکلوپرامید نیم ساعت قبل از خاتمه عمل تجویز گردیدند. بروز تهوع و استفراغ و نیاز به درمان در ریکاوری (1 ساعت اول بعد از عمل)، 1 تا 6 ساعت، 6 تا 12 ساعت و 12 تا 24 ساعت بعد از عمل در پرسشنامه ثبت گردید و توسط آزمون مجذور کای و Kruskall-wallis مورد تحلیل قرار گرفت. </p><p align="right"> <strong> یافتهها: </strong>از نظر میانگین سنی، وزن و جنس تفاوت معنیداری بین گروهها وجود نداشت. در گروه دارویی اندانسترون کمترین میزان تهوع و استفراغ بعد از عمل (8/7%) و کمترین میزان نیاز به داروی ضدتهوع و استفراغ (6/3%) مشاهده شد (در گروه دگزامتازون به ترتیب 6/12% و 5/13% و در گروه متوکلوپرامید 5/13% و 8/10%). </p><p align="right"> <strong> نتیجهگیری: </strong>کاربرد داروی اندانسترون در جراحی استرابیسم با کاهش بروز تهوع و استفراغ، کاهش بستری مجدد و هزینههای درمانی و رضایت بیشتر بیماران همراه است. </p><p align="right"> <strong> واژههای کلیدی: </strong>استرابیسم، تهوع و استفراغ بعد از عمل جراحی، اندانسترون، دگزامتازون، متوکلوپرامید </p><p align="right"> </p>
<p> <strong> Background and Objectives </strong>: Post operative nausea and vomiting (PONV) are important complications of surgeries causing an unpleasant experience for many patients and may lead to a delays in the discharge from hospital. Different medical and non-medical interventions are applied to prevent this problem. In this study, we compared the antiemetic effects of ondansetron, metoclopramide and dexamethasone in children undertaken strabismus surgey. </p><p> <strong> Materials and Methods: </strong>In this double-blind randomized clinical trial study , 111 children belonging to ASA class I and II between the age group of 2–15 years old and scheduled for elective surgery of strabismus were Ethical approval and written informed consent forms were obtained . The participants were then randomly divided into three groups ondansetron, metoclopramide and dexamethasone. Dexamethasone was injected at the beginning of the operations, and the other two medicines were injected 30 minutes prior to the end of surgery. In the recovery wards, the frequency of nausea and vomiting and any therapeutic interventions for each patient were recorded at the following times 1h, 1-6h, 6-12h, and 12-24h post-op. The obtained data was ultimately analyzed by Chi - squared and Kruskall statistical tests. </p><p> <strong>Results </strong><strong>: </strong>There was no significant differences among the three groups in terms of age, weight and sex. Patients in ondansetron group had the lowest frequency of PONV (7.8%) and the need for therapeutic interventions (3.6%).
