fa دقت تشخیصی نسخه فارسی ابزار سطح کارکرد حافظه (CDR-P) در تشخیص زودهنگام دمانس در سالمندان Diagnostic Accuracy of Persian Version of Clinical Dementia Rating (P-CDR) for Early Dementia Detection in the Elderly پرستاري Nursing پژوهشي Research  gte vml 1]><v:roundrect id="_x0000_s1026" style='position:absoluteleft:0text-align:left margin-left:5.45ptmargin-top:2.2ptwidth:449.5ptheight:392.7ptz-index:-1' arcsize="1475f"> <w:wrap anchorx="page"/> </v:roundrect><img alt="AWT IMAGE" width="602" height="526" src="file:///C:/DOCUME~1/ADMINI~1/LOCALS~1/Temp/msohtmlclip1/01/clip_image001.gif" v:shapes="_x0000_s1026" ><div style="direction: rtl ">چکیده</div> زمینه و هدف: در دوره سالمندی با توجه به پیامدهای دمانس، غربالگری آن دارای اهمیت است. از آنجا که ابزارهای مورد استفاده باید از حساسیت و دقت مطلوبی برخوردار باشند، مطالعه‌ای با هدف تعیین نقطه‌برش و دقت تشخیصی نسخه فارسی آزﻣﻮن ﺳﻄﺢ ﮐﺎرﮐﺮد ﺣﺎﻓﻈﻪ (P-CDR) در غربالگری دمانس در سالمندان انجام شد.<o:p /> <div style="direction: rtl ">مواد و روش‌ها: این مطالعه از نوع مقطعی است و در آن از روش گروه‌های شناخته‌ شده استفاده‌ شده است. حجم نمونه‌ شامل 150 سالمند بدون دمانس و 50 سالمند مبتلا به دمانس بود. نمونه‌گیری به ‌صورت هدفمند و در سال 1393 در شهر کاشان انجام شد. در این مطالعه مصاحبه بالینی و سی‌تی‌اسکن، به عنوان تست طلایی و P-CDR به عنوان آزمون تشخیصی منظور شد. داده‌ها با استفاده از پرسشنامه اطلاعات دموگرافیک، مقیاس افسردگی سالمندان (GDS-15) و </div><div style="direction: rtl ">P-CDR جمع‌آوری شد. از آزمون Mann–Whitney U برای مقایسه میانه نمرات CDR در دو گروه، منحنی راک برای تعیین نقطه برش و از پارامترهای ارزش تشخیصی برای تعیین دقت تشخیصی P-CDR در فاصله اطمینان 95% استفاده شد.</div> یافته‌ها: نمره CDR برای سالمندان غیرمبتلا و مبتلا به دمانس به ترتیب 69/1±82/0 و 76/1±36/8 بود (001/0>p). سطح زیر منحنی راک ابزار P-CDR معادل 968/0 بود (001/0>p)؛ با استفاده از شاخص Youden، نقطه برش 75/3 به‌عنوان بهترین نقطه برش ابزار P-CDR در نظر گرفته شد. در این نقطه، پارامترهای ارزش تشخیصی حساسیت، ویژگی، ارزش اخباری مثبت و منفی، نسبت درست نمایی مثبت و منفی، دقت کلی و نسبت شانس تشخیصی به ترتیب 1، 94/0، 88/0، 1، 0، 42/21، 96/0 و بی‌نهایت بود.<o:p /> نتیجه‌گیری: P-CDR ابزاری حساس و دقیق جهت سنجش وضعیت شناختی در سالمندان است. لذا در امر غربالگری دمانس و در برنامه پایش سلامت سالمندان می‌تواند مورد استفاده قرار گیرد.<o:p /> <div style="direction: rtl ">واژه‌های کلیدی: آزمون سطح کارکرد حافظه، دمانس، تست تشخیصی، حساسیت، ویژگی</div> Background and Objective: According to the complications of dementia, its screening is important in elderly stage. Dementia screening tests need to have acceptable sensitivity and specificity. This study aimed at determining the cut-off point of the Persian version of Clinical Dementia Rating (P-CDR) and testing its diagnostic accuracy in the early detection of dementia among the elderly.<o:p /> Materials and Methods: This cross-sectional study was conducted by using the known-groups method. The purposive sampling of 50 patients with age-related dementia and 150 the elderly without dementia was recruited in kashan, Iran, 2014. The clinical interviewing and the findings of CT-Scan were used as the reference standard test. The index test was the P-CDR. A demographic questionnaire, the Geriatric Depression Scale (GDS-15), and the P-CDR were used for data collection. The median of CDR scores in two known groups were compared using Mann-Whitney U test. The Receiver Operating Characteristic (ROC) analysis was conducted for determining the cut-off point. The diagnostic parameters were calculated for determining the diagnostic accuracy of the test with 95% confidence interval.<o:p /> Results: The cognitive state scores of the elderly with and without dementia differed significantly (8.36±1.76 vs. 0.82±1.69, respectively p< 0.001). The area under the ROC curve was 0.968 (p < 0.001). The Youden’s index showed that the optimal cut-off point for the test was 3.75. The sensitivity, specificity, positive and negative predictive values, positive and negative likelihood ratio, overall diagnostic accuracy, and diagnostic odds ratio of the test at the cut-off point of 3.75 were 1, 0.94, 0.88, 1, 0, 21.42, 0.96, and infinity, respectively.<o:p /> Conclusion: The P-CDR is a sensitive and precise test for assessing cognitive state in the elderly.
