fa تعیین دقت سونوگرافی و قانون نیگل در تخمین زمان زایمان زنان Gynecology پژوهشي Research مقاله پژوهشی مجله دانشگاه علوم پزشکی رفسنجان دوره ششم، شماره سوم، پاییز 1386، 170-163 تعیین دقت سونوگرافی و قانون نیگل در تخمین زمان زایمان دکتر راضیه دهقانی‌فیروزآبادی ، تینا بوترابی ، دکتر نعیمه طیبی دریافت مقاله:1/7/85 ارسال مقاله به نویسنده جهت اصلاح: 8/12/85 دریافت اصلاحیه از نویسنده: 18/4/86 پذیرش مقاله: 17/6/86 چکیده زمینه و هدف: تعیین سن حاملگی و تخمین زمان زایمان از فاکتورهای مهم در مراقبت دوران حاملگی است. قانون نیگل یک روش معمولی برای تعیین سن حاملگی و تخمین زمان زایمان (EDC) بر پایه LMP است که یک سال و هفت روز اضافه و 3 ماه کم می‌شود. امروزه سونوگرافی یکی از روش‌های معمول جهت تخمین تاریخ زایمان و سن حاملگی است. هدف ما از این مطالعه تعیین کردن دقت قانون نیگل و سونوگرافی جهت تخمین زمان زایمان می‌باشد. مواد روش‌ها: در این مطالعه ارزیابی آزمون تشخیصی، 260 زن باردار که جهت مراقبت دوران حاملگی به بیمارستان‌های شهید صدوقی یزد و مادر مراجعه کرده بودند، با یک توزیع مساوی در سه ماهه دوم و سوم، موارد مورد مطالعه قرار گرفتند و EDCآن‌ها بر اساس قانون نیگل و سونوگرافی به روش BPD وFL تخمین و با زمان واقعی زایمان مقایسه شد. یافته‌ها: اختلاف بین EDC بر اساس قانون نیگل و زمان واقعی زایمان 5- تا 8+ روز بود. این اختلاف، زمانی که EDC بر اساس سونوگرافی بود به 1- تا 18+ روز تغییر کرد. در سه ماهه دوم، میانگین به دست آمده برای قانون نیگل 21/2 روز و از طریق سونوگرافی 32/4 روز بود. در سه ماهه سوم، اعداد به دست آمده از طریق قانون نیگل 42/2 روز و با روش سونوگرافی20/9 روز با زمان واقعی زایمان اختلاف داشت. نتیجه‌گیری: نتایج به دست آمده بیانگر این مطلب است که قانون نیگل جهت تخمین زمان زایمان، دقت بیشتری نسبت به سونوگرافی داشته و دقت هر دو روش به ویژه سونوگرافی با افزایش سن حاملگی کاهش می‌یابد. واژه‌های کلیدی: زمان تخمینی زایمان، قانون نیگل، سونوگرافی، سن حاملگی <p>The Accuracy Determination of the Naegele’s Rule and Sonography for Estimating the Delivery Date</p><p> R. Naegle’s rule is a usual method for determining GA and EDC based on the LMP in which 7 days added and 3 months subtracted. Nowadays sonography is one of the routine methods for estimating date of delivery and gestational age. The aim of this study was to determine the accuracy of Naegele's rule and sonography for estimating of delivery date. Materials and Methods: In this analytic-descriptive study, a total of 260 pregnant women from the Yazd prenatal care clinic were equally divided into second and third trimester. The EDCs of these ladies were determined by Naegele's rule and sonographic BPD and FL measurement. The values of these two methods were then compared with actual onset of the spontaneous labor. Results: The difference between EDC based on the Naegele's rule and actual onset of labor was (-5) - (+8) days. However, this value changed to (-1) - (+18) when performed sonography was based on EDC. The derived means were 2.21 days for the Naegele's rule and 4.32 days for sonography at the second trimester, and at the third trimester , these figures were 2.42 days and 9.20 days respectively. Conclusions: The results suggest that the Naegele's rule is more accurate than sonography for the determination of EDC, and these methods especially sonography become less accurate with increasing gestational age.
