<p align="right"> </p><p align="right"><table cellspacing="0" cellpadding="0" width="100%"><tbody><tr><td><p> </p></td></tr></tbody></table></p><p align="right"><strong>چکیده </strong></p><p align="right"> <strong> زمینه و هدف: </strong>لوسمی لنفوسیتی مزمن ( CLL ) با تکثیر منوکلونال و تجمع لنفوسیتها و اغلب سلولهای <strong></strong>B (B-CLL<strong>)</strong> در خون و مغز استخوان مشخص میشود. اختلالات مرفولوژیک و عملی لنفوسیتهای T و منوکلونال بودن آنها در B-CLL مشخص شده است. این سلولهای تکثیر یافته ممکن است برای تشخیص پاتوژنهای خاص اختصاصی باشند و سیتومگالوویروس (CMV) محتملترین عامل درگیر در این پدیده در B-CLL است. عفونت CMV پاسخ ایمنی شدیدی در افراد مبتلا به نقص ایمنی ایجاد میکند. در این مطالعه امکان ارتباط افزایش لنفوسیتهای T در B-CLL با CMV و پاسخ اختصاصی سلولهای <sup>+</sup> CD4 و<sup>+</sup> CD8 نسبت به آن مورد تحقیق قرار گرفت. </p><p align="right"> <strong> مواد و روشها </strong>: این مطالعه از نوع مقطعی و گروه مورد بررسی 41 بیمار CLL دارای سرولوژی مثبت CMV و 35 فرد سالم دارای سرولوژی مثبت CMV در همان گروه سنی به عنوان کنترل بود. میزان لنفوسیتهای اختصاصی از زیرگروه <sup><sup>+ </sup>CD4 و <sup>+</sup> CD8 به ترتیب با استفاده از سیتوکینهای داخل سلولی پس از تحریک آنتیژنیک و با استفاده از تترامر مشخص گردید. </sup></p><p align="right"> <strong> یافتهها </strong>: پاسخ ایمنی اختصاصی نسبت به CMV در بیماران CLL با میانه 99/10 و 11 به ترتیب برای سلولهای تولید کننده اینترفرون- گاما (γ (IFN- و فاکتور نکروز کننده تومور-آلفا ( (TNF-α به صورت معنیداری بیش از گروه کنترل با میانه 3/4 و 6/4 بود (0001 p= / برای γ IFN- و 003/0 p= برای α- TNF ). پاسخ ایمنی سلولهای CD8<sup>+</sup> نیز در بیماران بیش از گروه شاهد بود (03/0 p= ) . بین پاسخ ایمنی سلولهای <sup>+</sup> CD4 و<sup>+</sup> CD8 همبستگی مثبت معنیداری مشاهده شد (009/0 p= ). </p><p align="right"> <strong> نتیجهگیری </strong>: این نتایج تأثیر عمیق CMV و پاسخ اختصاصی نسبت به آن توسط سلولهای T را در تغییرات سلولهای T در CLL نشان میدهد. افزایش تعداد لنفوسیتهای اختصاصی CMV ممکن است عامل افزایش لنفوسیتهای T در این بیماران باشد. </p><p align="right"> <strong> واژههای کلیدی </strong>: لوسمیلنفوسیتی مزمن، سیتومگالوویروس، لنفوسیت T ، اینترفرون گاما، فاکتور نکروز کننده تومور آلفا </p><p align="right"> </p>

Moss </strong><a name="_ftnref2"><u>[2]</u> </a><strong></strong></p><p> <strong> </strong></p><p> <strong> Received: 10/09/07 Sent for Revision: 05/03/08 Received Revised Manuscript: 06/12/08 Accepted: 30/12/08 </strong></p><p> <strong> Background and Objective: </strong>Chronic lymphocytic leukemia (CLL) is characterized by a monoclonal proliferation of lymphocytes mainly B cells (B-CLL) in the blood and bone marrow. Morphological and functional abnormalities of T cells and monoclonality of them have been documented in CLL. Such expanded cells may be specific for recognition of pathogens. Cytomegalovirus (CMV) is most likely involved in this phenomenon in CLL. CMV infection causes a high level of immune response in immunodeficient patients. In this study, the association between expanded T cells and the immune responses of CMV-specific CD4<sup>+</sup> and CD8<sup>+</sup> cells in B-CLL was investigated.<strong></strong> </p><p> <strong> Matherials and Methods </strong>: This was a cross sectional study and the study group were 41 CMV seropositive B-CLL patients and 35 CMV seropositive healthy donors (control group). The level of CMV-specific CD4<sup>+</sup> and CD8<sup>+</sup> cells immune responses were detected by intracellular cytokine response to antigenic activation and CMV tetramer respectively. </p><p> <strong> Results: </strong>The level of CMV-specific CD4<sup>+</sup> cells responses was significantly higher in patients with a median of 10.99% and 11% for IFN-γ and TNF-α producing CD4+T cells respectively than age-matched controls with a median of 4.3% and 4.6 (p= 0.0004 for IFN-γ and p= 0.003 for TNF-α). The level of CMV-specific CD8<sup>+</sup> cells immune responses was also higher in patients compared to control group (p= 0.03 for NLV). There was a positive correlation between the level of CD4<sup>+</sup> and CD8<sup>+ </sup>cells immune responses (p=0.009). </p><p> <strong> Conclusions </strong>: These results show the deep influence of CMV infection and the immune response to it in T-cell alteration in CLL patients.
