fa مقایسه هزینه‌های اعمال جراحی گلوبال با تعرفه‌های مصوب دولتی در بیمارستان تک تخصصی زنان و زایمان نیک‌نفس رفسنجان در سال 1398: یک مطالعه توصیفی آمار و اپيدميولوژي Statistics& Epidemiology پژوهشي Research <strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">زمینه و هدف: </span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">محاسبه هزینه&shy;های خدمات بستری مشمول تعرفه گلوبال و مقایسه آن با تعرفه مصوب در بیمارستان&shy;ها از نظر سود و زیان بیمارستان&shy;ها بسیار حائز اهمیت است. این پژوهش با هدف تعیین و مقایسه هزینه&shy; صورت حساب&shy;های اعمال جراحی گلوبالبا تعرفه&shy;های مصوب آن&shy;ها در بیمارستان تک تخصصی زنان و زایمان نیک&shy;نفس دانشگاه علوم پزشکی رفسنجان انجام گرفت.</span></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">مواد و روش&shy;ها:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"> در این مطالعه توصیفی، جامعه پژوهش شامل کلیه صورت حساب&shy;های موجود در پرونده&shy;های بستری مشمول تعرفه گلوبال در سال 1398 در بیمارستان نیک&shy;نفس رفسنجان بود. داده&shy;های مورد نیاز از صورت &shy;حساب&shy;&shy;های بیماران جمع</span></span><span dir="LTR">‌</span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">آوری و با استفاده از آمار توصیفی</span></span> <span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">و نرم افزار </span></span><span dir="LTR">SPSS</span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"> نسخه20 تحلیل گردید.</span></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">یافته&shy;ها:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"> در سال 1398 از مجموع خدمات بستری مربوط به گروه زنان و زایمان، 6 خدمت مشمول تعرفه گلوبال انجام شد. مقایسه میانگین هزینه&shy;های انجام شده در این خدمات با تعرفه مصوب آن&shy;ها نشان داد در تمامی آن&shy;ها، هزینه&shy;های بیمارستان بین 74/10 درصد تا 54/20 درصد بیش‌تر از تعرفه گلوبال مصوب بوده است. بیش‌ترین اختلاف (54/20 درصد) مربوط به جراحی سزارین و کم‌ترین آن (74/10 درصد) مربوط به جراحی هیستروکتومی بود. </span></span><span dir="LTR"></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">نتیجه&shy;گیری:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"> نتایج این پژوهش نشان داد خدمات گلوبال بیمارستان نیک&shy;نفس رفسنجان دارای توجیه اقتصادی نبوده و تعرفه</span></span><span dir="LTR">‌</span><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">های گلوبال منجر به زیان بیمارستان می&shy;گردند. لذا پیشنهاد می&shy;گردد که مسئولین بیمارستان تمرکز بیش‌تری به مدیریت هزینه&shy;های خدمات گلوبال داشته باشند و نیز تعرفه&shy;های مصوب متناسب با رشد تورم و با در نظر گرفتن شرایط اقتصادی بیماران، مورد بازبینی قرار گیرند.</span></span><span dir="LTR"></span><br> <strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;">واژه&shy;های کلیدی:</span></span></strong><span style="font-family:B Nazanin;"><span style="font-size:12.0pt;"> مقایسه هزینه&shy;ها، تعرفه گلوبال، دانشگاه علوم پزشکی رفسنجان</span></span><span dir="LTR"><span style="font-size:14.0pt;"></span></span><br> &nbsp;<span dir="LTR"></span> <strong>Background and Objectives:</strong> It is very important to calculate the costs of inpatient services covered by the global tariff and compare it with the approved tariff in hospitals in regard to the financial profit and loss. In this regard, this study was conducted with the aim of comparing the costs of global inpatient services bills with their approved tariffs in Niknafs Obstetrics and Gynecology Hospital in Rafsanjan.<br> <strong>Materials and Methods:</strong> In this descriptive study, the population included all the invoices of hospitalized cases subjected to the global tariff in 2019 in Niknafs Hospital in Rafsanjan. The required data were collected from the patients&#39; accounts and analyzed using descriptive statistics and SPSS 20 software.<strong><span dir="RTL"></span></strong><br> <strong>Results:</strong> In 2019, out of the total inpatient services related to the obstetrics and gynecology group, 6 services subjected to the global tariffs were performed in Niknafs Obstetrics and Gynecology Hospital in Rafsanjan. A comparison of the average costs incurred in these services with their approved tariffs showed that in all of them, hospital costs were between 10.74% and 20.54% higher than the approved global tariffs. The highest difference (20.54%) was related to cesarean section, and the lowest (10.74%) was related to hysterectomy.<strong><span dir="RTL"></span></strong><br> <strong>Conclusion:</strong> The results of this study showed that the global services of this hospital have no economic justification and global tariffs lead to losses of the hospital.
