fa تجزیه و تحلیل ژنتیکی ناحیه D-loop توالی DNA میتوکندری در بیماران ایرانی مبتلا به پولیپوز آدنوماتوز خانوادگی: یک مطالعه مورد-شاهدی Genetic Analysis of D-Loop Region of Mitochondrial DNA Sequence in Iranian Patients with Familial Adenomatous Polyposis (FAP): A Case-Control Study انكولوژي Oncology گزارش مورد Case Report <a name="_Hlk113374001">زمینه و هدف: </a>پولیپوز آدنوماتوز خانوادگی (Familial adenomatous polyposis; FAP) یک اختلال ارثی و شکل نادری از سرطان کولورکتال است. این بیماری در هر دو جنس به طور مساوی ظاهر می‌شود و بروز آن بیشتر در دهه دوم یا سوم زندگی است. جهش‌ها و تغییرات ژنوم میتوکندری مخصوصاً در ناحیه D-loop با انواع تومورهای انسانی گزارش شده است. اما نقش دقیق این جهش‌ها در بیماری‌زایی و پیشرفت تومورها هنوز مشخص نیست. هدف مطالعه حاضر تعیین وقوع جهش‌های نوکلئوتیدی mtDNA در ناحیهD-loop  و به ویژه در جایگاه‌های بسیار متغیر (Hypervariable region; HVR)، در بیمارانFAP  است. مواد و روش‌ها: در این مطالعه مورد-شاهدی، 56 بیمار FAP از نظر وجود جهش در ناحیهD-loop  میتوکندری با استفاده از تکنیک‌های Touchdown PCR، پلی‌مورفیسم کنفورماسیون تک رشته‌ای (Single-stranded conformation polymorphism; SSCP) و آنالیز تعیین توالی بررسی شدند. از آنالیز آماری فیشر برای یافتن ارتباط آماری این تغییرات نوکلئوتیدی با بیماری FAP استفاده شد. یافته‌ها: 19 تغییر نوکلئوتیدی در ناحیهD-loop  ژنوم میتوکندری بیماران مبتلا به علائم FAP شناسایی شد. دو مورد از این تغییرات نوکلئوتیدی (C16335T و C16352T) جدید بودند و قبلاً در هیچ بیماری گزارش نشده بودند. نتایج آنالیز‌ آماری جهش‌های مشاهده شده، تفاوت آماری مهمی را در بین گروه بیماران و گروه کنترل نشان داد (05/ 0P<). نتیجه‌گیری: یافته‌های این مطالعه همبستگی مثبت جهش‌های mtDNA در ناحیهD-loop  در تمام موارد FAP در مقایسه با گروه‌های کنترل همسان نشان‌ می‌دهد. بنابراین، جهش در توالی‌های غیرکدکننده میتوکندری ممکن است با تأثیر بر فرآیند بیان ژن‌های میتوکندریایی منجر به تولید پروتئین‌های معیوب در زنجیره تنفسی شوند. واژه‌های کلیدی: DNA میتوکندریایی، D-loop، بد‌خیمی، سرطان کلورکتال، پولیپوز آدنوماتوز خانوادگی   <a name="_Hlk114425361">Background and Objectives:</a> Familial adenomatous polyposis (FAP) is an inherited disorder and a rare form of colorectal cancer. This disease appears equally in both sexes and its occurrence is more in the second or third decade of life. Mutations and alterations of the mitochondrial genome, especially the D-loop region, have been reported in various human tumors. But the exact role of these mutations in the pathogenesis and progression of tumors is still unclear. The purpose of the present study was to investigate the occurrence of mtDNA nucleotide mutations in the D-loop region, especially the hypervariable regions (HVR) in FAP patients. Materials and Methods: In this case-control study, 56 FAP patients were investigated for mutations in the mitochondrial D-loop region by Touchdown PCR, single-stranded conformation polymorphism (SSCP), and sequencing analysis. Fisher's exact statistical tool was used to find the relationship between these nucleotide changes and the risk of FAP disease. Results: Nineteen nucleotide changes were identified in the D-loop region of patients with FAP symptoms. Two of these nucleotide changes (C16335T and C16352T) were novel and not previously reported in any disease. The results of the statistical analysis of observed mutations showed a statistically significant difference between the patient's group and the control group (p<0.05). Conclusion: The findings of this study indicate a positive correlation of mtDNA mutations in the D-loop region in all FAP cases compared to the matched control groups. Therefore, the mutation in mitochondrial non-coding sequences may lead to the production of defective proteins in the respiratory chain by affecting the expression process of mitochondrial genes.
