fa فراوانی تظاهرات کلیوی و برخی عوامل مرتبط با آن در بیماران مبتلا به لوپوس مراجعه‌کننده به کلینیک روماتولوژی رفسنجان در سال 1401: یک مطالعه توصیفی Prevalence of Renal Manifestations and Associated Factors in Patients with Systemic Lupus Erythematosus Referred to the Rheumatology Clinic of Rafsanjan in 2022: A Descriptive Study ارولوژی Urology پژوهشي Research زمینه و هدف: لوپوس اریتماتوز سیستمیک (Systemic lupus erythematosus; SLE) یک بیماری خودایمنی مزمن است که ارگان‌های مختلف بدن ازجمله کلیه‌ها را تحت تأثیر قرار می‌دهد. این مطالعه با هدف تعیین فراوانی تظاهرات کلیوی و عوامل مرتبط با آن در بیماران SLE انجام شد. مواد و روش‌ها: در این مطالعه توصیفی-مقطعی در سال 1401، 187 بیمار SLE بررسی شدند. اطلاعات جمعیتی، یافته‌های بالینی و آزمایشگاهی جمع‌آوری شد. عملکرد کلیه با استفاده از نرخ فیلتراسیون گلومرولی تخمین زده شده <a name="_Hlk189897618">(</a>estimated Glomerular Filtration Rate; eGFR) ارزیابی شد. از تحلیل رگرسیون لجستیک به‌منظور تعیین عوامل مرتبط با بیماری کلیوی خفیف تا متوسط استفاده شد. یافته‌ها: 7/95 درصد (179 نفر) از بیماران زن بودند. 2/34 درصد (64 نفر) از بیماران بیماری کلیوی خفیف تا متوسط داشتند. بیماران با اختلال عملکرد کلیوی به‌طور معناداری مسن‌تر بودند (001/0>P) و میانگین سطوح BUN، کراتینین و فشارخون سیستولیک (001/0>P) بالاتری داشتند. نفریت لوپوسی در این گروه شایع‌تر بود (013/0=P). تفاوت معناداری در سایر تظاهرات بالینی، دموگرافیک و آزمایشگاهی بین دو گروه مشاهده نشد (05/0<P). در تحلیل چندمتغیره، افزایش سن و کراتینین  بهطور مستقل با افزایش خطر بیماری کلیوی مرتبط بودند (001/0>P). نتیجه‌گیری: مطالعه حاضر نشان داد حدود یک‌سوم بیماران مبتلا به SLE، شواهدی از اختلال عملکرد کلیوی خفیف تا متوسط داشتند که افزایش سن و سطح کراتینین سرم عوامل خطر مستقل در این زمینه هستند. بنابراین، به‌نظر می‌رسد نظارت منظم بر عملکرد کلیوی در بیماران SLE، اهمیت ویژه‌ای دارد. واژه‌های کلیدی: اریتماتوز، نفریت لوپوس، بیماری مزمن کلیوی، روماتولوژی   <a name="_Hlk114425361">Background and Objectives: </a>Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that affects various organs of the body, including the kidneys. This study aimed to determine the prevalence of renal manifestations and associated factors in patients with SLE. Materials and Methods: In this descriptive cross-sectional study carried out in 2022, 187 patients with SLE were evaluated. Demographic data, clinical, and laboratory findings were collected. Kidney function was assessed using the estimated glomerular filtration rate (eGFR). Logistic regression analysis was used to identify factors associated with mild to moderate kidney disease. Results: 95.7% (n=179) of the patients were female. 34.2% (n=64) of patients had mild to moderate kidney disease. Patients with impaired kidney function were significantly older (p<0.001) and had higher mean levels of BUN, creatinine, and systolic blood pressure (p<0.001). Lupus nephritis was more common in this group (p=0.013). No significant differences were observed between the two groups regarding other demographic, clinical, or laboratory findings (p>0.05). In multivariate analysis, increased age and serum creatinine levels were independently associated with a higher risk of kidney disease (p<0.001). Conclusion: The findings of current study showed that approximately one-third of SLE patients had evidence of mild to moderate renal impairment. Advanced age and elevated serum creatinine were identified as independent risk factors. Therefore, regular monitoring of kidney function in patients with SLE appears to be of critical importance.
