Rezaeian   همکاری (Collaboration) در پژوهش‌های علوم سلامت، جزء ضروری برای به موفقیت رسیدن چنین پژوهش‌هایی است. قبلاً در یکی از سخنان سردبیری مجله دانشگاه به مقالاتی اشاره کردیم که ممکن است تعداد نویسندگان آن‌ها تا هزار نفر هم بالا رفته چرا که بر اساس شبکه وسیعی از همکاری‌های بین‌المللی به انجام رسیده و چاپ شده‌اند [1]. راتکوف Rutkove در فصل سیزدهم از کتاب خود، همکاری را به موقعیتی اطلاق می‌کند که دو همتا (Peer) برای انجام یک پژوهش، با هم‌دیگر مشارکت می‌نمایند. بنابراین، رابطه بین استاد و دانشجو و یا دستیار پژوهش با پژوهشگر ارشد، در قالب تعریف چنین همکاریی نمی‌گنجد. راتکوف معتقد است بر اساس این تعریف، سه نوع همکاری به شرح زیر قابل رخ دادن است. نخست، همکاری شما در موضوع پژوهشی مورد علاقه‌تان با یک نفر همکار هم رشته از دانشگاه و یا محل خدمت خودتان. دوم، همکاری شما در موضوع پژوهشی مورد علاقه‌تان با یک نفر همکار هم رشته از سایر دانشگاه‌ها. سوم، همکاری شما با یک نفر همکار غیر هم رشته پیرامون یک موضوع پژوهشی خاص [2]. راتکوف هم‌چنین، اعلام می‌دارد که اگر چه هر سه نوع همکاری بسیار ضروری است، اما همکاری نوع سوم است که می‌تواند نتایج شگرفی را از خود بر جای بگذارد. برای مثال، وی به همکاری خودش به عنوان یک محقق علوم سلامت با برخی از اساتید انستیتو تکنولوژی ماساچوست The Massachusetts Institute of Technology (MIT) اشاره می‌کند و این‌که چگونه تفاوت در بین دیدگاه‌ها و روش‌های دانشمندان یک انستیتو تکنولوژی با دانشمندان علوم سلامت، می‌تواند به دستاوردهای علمی مهمی بینجامد [2]. راقم این سطور نیز در این سخن سردبیری، به ضرورت انجام همکاری در پژوهش‌های علوم سلامت به ویژه همکاری‌هایی که از مرزهای یک کشور فراتر رفته و دانشمندان از کشورهای مختلف و با تخصص‌های مختلف را گرد هم می‌آورد، تأکید می‌نماید. از آنجایی‌که مشکلات مربوط به سلامت چند بعدی بوده و نیازمند بررسی‌های همه جانبه از دیدگاه‌های مختلف و فرهنگ‌های مختلف می‌باشد، از همین‌رو، همکاری‌های بین‌المللی می‌تواند پاسخ‌هایی دقیق‌تر به مشکلات سلامت جامعه را ارائه نماید [3]. برای نمونه، به منظور بررسی علمی تأثیرات عالمگیری کووید-19 بر روی رفتارهای خودکشی، گروهی با عنوان همکاری پژوهشی بین المللی پیشگیری از خودکشی کوید-19 International COVID- 19 Suicide Prevention Research Collaboration (ICSPRC) تشکیل شده است. این گروه بین‌المللی از دانشمندان جهان، تا کنون نتایج همکاری‌های خود را در دو نشریه معتبر به چاپ رسانیده که مورد استقبال گسترده سایر محققین علوم سلامت قرار گرفته است [5-4].   References   [1] Rezaeian M. The aim of this study was to investigate the geographical epidemiology of suicide (completed- attempted) in the townships covered by Mazandaran University of Medical Sciences during the years 2012 - 2016. Materials and Methods: In this ecological study, information was collected using the checklist of the monthly report of the suicide prevention program in the hospitals of Mazandaran University of Medical Sciences. Spatial correlation of suicide age-sex groups was calculated using Moran's index (I). Data were analyzed using descriptive statistics and Chi-square test. Results: 376 cases (1.67%) had completed suicides and 22089 cases (98.33%) had suicide attempts. Of the total number of people who committed suicide, 68.5% (15389 cases) were urban and 31.5% (7076 cases) were rural. The highest number of suicide attempts was reported from Amol city 3549 cases (16.07%) and the highest number of completed suicides was reported from Sari city 65 cases (17.29%). In cases of completed suicide, the most common method of suicide in Sari and Amol counties was the use of rice pills and in the cities of Gulogah and Nowshahr was hanging. In cases of suicide attempts, the use of psychiatric drugs was the most reported in Ghaemshahr and Amol counties. Conclusion: According to the geographical pattern, the number of suicides (completeed and attempted) was higher in most age and sex groups in densely populated cities. The use of rice pill consumption method in completed suicide and violent and lethal methods was high in some areas of the province. So, the aim of the present study was to investigate the effect of one course of intense interval training on serum levels of vitamin D, HRV and lung function in male smokers. Materials and Methods: In this quasi-experimental study 20 sedentary male voluntarily participated. They were randomly divided into experimental (n=10) and control (n=10) groups. The experimental group participated in an intense interval training program for six weeks and control group did no exercise training. HRV and pulmonary function were assessed by spirometry device and serum levels of vitamin D 48 hours before the first session and 48 hours after the end of last session training. Collected data were analyzed by paired t-test and Multivariate analysis of covariance for within group and between group comparisons, respectively. Results: The results showed a significant increase in HRV values and pulmonary function and low frequency waves to high frequency waves in the training group compared to the pretest and also compared to the control group (p<0.05). But no significant difference was observed in serum levels of vitamin D both in within group and between group comparisons (p<0.05). Conclusion: It can be concluded that six weeks of intense interval training, improves function of the autonomic nervous system and pulmonary function in male smokers. DashtBozorgi Z. 1 6 2021 20 3 297 312 19 02 2021 27 04 2021 Background and Objectives: Depressed patients face problems in field of life expectancy and psychological health. Therefore, the present research aimed to compare the effectiveness of acceptance and commitment therapy and well-being therapy on life expectancy and psychological health in elderly nonclinical depressed patients. Materials and Methods: This study was a semi-experimental study with pretest and posttest design with control group. The research population was elderly nonclinical depressed patients referred to the psychological services clinics and centers of Ahvaz city in the spring season of 2020. 75 people were selected by purposive sampling method and randomly replaced into three equal groups. The experimental groups were separately trained 10 sessions of 90 minutes with the methods of acceptance and commitment therapy and well-being therapy, and the control group did not receive any training. The research instruments were the Scales of Yasavage and Sheikh elderly depression, Snyder and et al. life expectancy and Najarian and Davodi psychological health. Data were analyzed by chi-square, multivariate analysis of covariance and Bonferroni post hoc tests. Results: The findings showed that groups were not significantly different in terms of gender, age and education (p>0.05). Also, both methods of acceptance and commitment therapy and well-being therapy in comparison with the control group led to a significant improvement in the life expectancy and psychological health (p<0.001), but there was no significant difference between two methods in improving the variables (p>0.05). Conclusions: Based on the results, therapists and health experts can use both methods of acceptance and commitment therapy and well-being therapy to improve health related traits especially the life expectancy and psychological health improvement. Mirzaee V. 1 6 2021 20 3 313 324 06 12 2020 09 05 2021 Background and Objectives: Proton pump inhibitors (PPIs) have an important role in stabilizing clots caused by upper gastrointestinal bleeding. The aim of this study was performed to compare and determine the effectiveness of the minimum and maximum dose of pantoprazole in the treatment of patients with hemorrhagic gastric peptic ulcer, referred to Rafsanjan Ali ebn Abitaleb Hospital. Materials and Methods: This randomized clinical trial was performed on 104 patients with upper gastrointestinal bleeding in Rafsanjan Ali ebn Abitaleb Hospital in 2016-2017. Patients were randomly divided into two equal groups. Group A was given a high dose of pantoprazole (80 mg) and group B was given a low dose (40 mg). For gender and age matching, the two groups were compared (p>0.05). Data were analyzed using descriptive statistics (frequency, percentage, mean and standard deviation) and inferential statistics (independent t-test, chi-square test or Fisher's exact test).  Results: The mean and standard deviation of patients' age in group A were 52.96±22.79 years and in group B 52.51± 23.18 years (p=0.921). No significant difference was observed between the sexes in the two groups (p=0.823). PPI dose was not significantly associated with mortality rate, hemorrhage, hospitalization days, drop in hemoglobin, need for surgery (p>0.05). Conclusion: In this study, the efficacy of high-dose pantoprazole compared with low-dose in clinical outcomes including mortality, need for surgery, hemorrhage, number of hospitalization days, and hemoglobin drop did not differ between the two groups. Therefore, for the health of patients at the optimal dose, clinical studies with a larger sample size are recommended. Talebizadeh N. rafsanjan medical university rafsanjan medical university Rafsanjan Education Management rafsanjan medical university rafsanjan medical university rafsanjan medical university rafsanjan medical university 1 6 2021 20 3 325 338 05 12 2020 16 05 2021 Background and Objectives: According the importance of iodine monitoring in the prevention of iodine deficiency disorders, this study aimed to evaluate the clinical iodine and goiter deficiency in 7-12 year-old students in Rafsanjan in 2018-19. Materials and Methods: The statistical population of this cross-sectional study included all students of 7-12 years old who were covered by Rafsanjan Education Management in 2018. From 21218 students aged 7 to 12 years living in the city, 1203 students (601 boys and 602 girls) based on inclusion criteria were enrolled in the study. After obtaining informed consent from the parents, the students were examined clinically by an internal trained resident. In this study, the presence and severity of goiter were evaluated based on the World Health Organiaztion classification. Then, 10 ml of the morning urine sample of 10% of all samples was taken to measure iodine by acid digestion method. Results: The mean and standard deviation of age of participants was 9.48±1.71. In addition, mean and standard deviation of urinary iodine was 183.33±80.52 Ug / dl. In the study of the prevalence of goiter in the community, 87.1% (1047 samples) had no goiter, 9.7% (117 samples) had D1 grade and 3.2% (39 samples) had D2 grade. It was also found that the prevalence of goiter was not statistically significant in terms of age and sex. Conclusion: The findings of this study indicate that the iodine deficiency control program and consumption of iodized salt has caused a significant decrease in the prevalence and severity of goiter, and also adequacy of the received iodine, given a marked increase in urinary iodine excretion in Rafsanjan city. Although different complications have been reported following coronavirus infection, lung involvement is the most important cause of death. In many cases, the disease eventually subsides; however, major short- and long-term complications may persist in a minority. Lung fibrosis, secondary bacterial and fungal infections, cardiovascular involvement such as myocarditis, pericarditis and pulmonary artery emboli may occur in the acute phase or present as a sequel. Different imaging modalities, especially chest CT scan, have an essential role in the initial diagnosis and can be used to monitor the clinical course of the disease, evaluate the response to treatment, and detect possible complications. This article reviews the major pulmonary and cardiovascular complications caused by COVID-19 infection, focusing on their radiologic manifestations. Familiarity with such diagnostic imaging hallmarks is crucial for early diagnosis and optimal patient management and prevention or reduction of mortality. The aim of this study was to evaluate the coverage of influenza vaccine and its related factors in the staff of Birjand teaching hospitals. Materials and Methods: In this cross-sectional study, a researcher-made questionnaire including demographic, ward and hospital characteristics of the workplace, regular and annual use of seasonal influenza vaccine, staff knowledge and attitude questions, possible reasons for using or not using the vaccine were completed by 233 participants. The data was analyzed by using chi-square and Mann-Whitney U tests. Results: 8.8 percent of the staff had annual vaccine use and 48.5percent had at least one seasonal influenza vaccine. The mean score of knowledge and attitude among the hospital personnel regarding influenza vaccination was 5.5±1.9 out of 9. Conclusion: Influenza vaccination coverage is low in educational hospitals staff in Birjand. Therefore, there is a need for further education to increase staff awareness about the importance of vaccination. Keywords: Influenza, Vaccination, Health care personnel   Funding: None declared. Fadaei A.M. m sh 1 6 2021 20 3 365 368 28 03 2021 03 05 2021 سردبیر محترم کیفیت و عدم دسترسی به آب به عنوان یک چالش اساسی در سراسر جهان و ایران در حال ظهور است و نیاز فزآینده­ای به پایش کیفیت آب در طیف وسیعی از منابع شامل، آب آشامیدنی، فاضلاب شهری و صنعتی تا آب‌های محیطی(رودخانه­ها، دریاچه­ها، آب­های زیرزمینی و اقیانوس­ها) وجود دارد [1]. موضوعات موجود در مورد کیفیت آب پیچیده است و شامل طیف گسترده­ای از آلاینده­های شیمیایی و بیولوژیکی و نو­ظهور است که بسیار نگران­کننده هستند [2]. فعالیت‌های پایش می‌تواند به درک، حفاظت و بهبود زیستگاه‌های آبی و تجزیه و تحلیل داده‌های کیفیت آب جهت کَمی کردن تغییرات محیطی و توسعه بهترین شیوه‌های مدیریتی برای تصمیمات آگاهانه، کمک کند. بنابراین، شبکه پایش کیفیت آب (water quality monitoring network:WQMN) عنصر اصلی یا کلیدی برای مدیریت و حفاظت از اکوسیستم آبی است [3]. پایش متداول و سنتی کیفیت آب شامل نمونه­برداری در محل و انتقال نمونه­ها به آزمایشگاه و آزمایش تعیین خصوصیات فیزیکی، شیمیایی و بیولوژیکی آب است [2]. سیستم‌های پایش کیفی آب سنتی به علت قابلیت تغییرپذیری بالای مکانی-زمانی پارامترهای فیزیکوشیمیایی یا میکروبی آب، دارای محدودیت‌های زیادی هستند. علاوه بر این، چندین عامل خطا وجود دارد که می‌توانند بر رویکرد پایش دستی آب بصورت سنتی تأثیر بگذارند. این خطاها شامل: خطاهای انسانی، خطای مربوط به تجهیزات آزمایشگاهی و سایر خطاها می­باشد [5-4]. مدیریت مدرن آب به شناسایی مطمئن­تر (صحت بالا) و سریع‌تر آلاینده‌ها نیاز دارد تا پاسخ به موقع و بجا دهد. پایش در زمان واقعی (Real-time monitoring) می‌تواند پاسخ یا واکنش سریع به نگرانی‌های مربوط به کیفیت آب را که ناشی از آلودگی منابع طبیعی یا عمدی است، فراهم کند و بیش‌ترین حفاظت از سلامت عمومی را میسر ‌سازد. در حالت ایده­آل، همه پایش کیفیت آب باید به صورت Real-time انجام شود، تا بتوان دقیق­ترین و صحیح­ترین قضاوت در خصوص کیفیت آب را ارائه داد [2]. برای ارزیابی تغییرات کیفی آب، شناسایی روندها (trends) و تعیین وضعیت کیفیت آب و اکوسیستم آبی، شناسایی آلودگی‌های نوظهور و آلاینده­ها و دستیابی سریع جهت غربالگری (screening) آب برای مواد سمی و عوامل بیماری­زا می توان از اطلاعات Real-time استفاده کرد. کاربرد مهم دیگر آن، پایش و بهینه­سازی فرآیندهای تصفیه آب و فاضلاب برای اطمینان از انطباق با استانداردهای کیفی آب است [3-2]. سیستم­های هوشمند پایش کیفی آب با استفاده از حسگرهای (Sensors) می­توانند پارامترهای کیفی آب شامل: pH، اکسیژن محلول (Dissolved Oxygen; DO)، دما، پتانسیل اکسیداسیون-احیاء (Oxidation Reduction Potential; ORP)، BOD، EC، TDS و کدورت بصورت Real time و مداوم اندازه­گیری کنند که برای تشخیص سریع آلودگی­ها و ایجاد پاسخ سریع به آلودگی­ها اهمیت حیاتی دارند. سیستم­های هوشمند پایش کیفی آب براساس شبکه حسگر بی­سیم (Wireless Sensor Network; WSN) و (Internet of Things; IOT) کار می­کنند و می­توانند داده­های کیفیت آب به صورت Real time و پیوسته از راه دور انتقال و پردازش کنند و محدودیت­های مربوط به پایش کیفیت آب را به صورت متداول و سنتی را ندارند. IOT به اشیا و تجهیزات محیط پیرامون‌مان که به شبکه اینترنت متصل شده و توسط اپلیکیشن‌های موجود در تلفن‌های هوشمند و تبلت قابل کنترل و مدیریت هستند، گفته می­شود. IoT به طور کلی شامل یک کنترل‌کننده، سنسور و یک برنامه (Application) برای نمایش داده می‌باشد. سنسورها دستگاه‌های متصل به کنترل ‌کننده هستند که برای ثبت مقادیر مانند مقدار PH، هدایت الکتریکی، اکسیژن، شوری و غیره استفاده می‌شوند. حسگرها مقدار محاسبه شده را جمع می­کنند و آن را به کنترل کننده منتقل می­کنند. کنترل ‌کننده یک کامپیوتر با اندازه کوچک، با قابلیت اتصال به شبکه و اجرای برنامه‌ها است. آن می‌تواند برای دریافت مقادیر از سنسورها برنامه‌ریزی شود و مقادیر را برای ذخیره‌سازی و تجزیه و تحلیل به اینترنت منتقل کند. یک برنامه اساساً برنامه‌ای است که در اینترنت برای دریافت مقادیر از کنترل‌ کننده دسته‌بندی شده است و در نمایش‌گر کاربر نمایش داده می‌شود [6]. شبکه­های WSN شامل گره­های حسگر(sensor nodes) خودکار، خودپیکربندی و  ایستگاه پایه گره­ها (یا مستغرق) است که قادر به اندازه‌گیری برخی پدیده فیزیکی و انتقال اطلاعات به‌دست‌آمده به یک دفتر مرکزی هستند. گره‌های حسگر در محل‌های نمونه‌گیری مختلف پیکره آبی، مستقر می‌شوند. ارتباط از راه دور بین گره­های حسگر و دفتر مرکزی، با استفاده از یک فرستنده رادیویی کم مصرف به منظور به حداکثر رساندن طول عمر گره های حسگر انجام می­شود. داده‌های جمع‌آوری شده توسط گره‌های حسگر از طریق دروازه‌ای به ابر یا پایگاه داده با استفاده از یک شبکه ارتباطی ارسال می‌شود. داده‌های سنجش شده از این حسگرها در یک پورتال وب در زمان واقعی در دسترس قرار می‌گیرند و ذینفعان را قادر می‌سازد تا در فرآیندهای مدیریت کیفیت آب به طور مؤثر مداخله کنند [7]. با توجه به محدودیت منابع آب شیرین در کشور، ضرورت دارد که مسئولین و تصمیم سازان مرتبط با مدیریت منابع آب استفاده حداکثری از سیستم­های هوشمند پایش کیفی آب جهت پایش و کنترل منابع آب در دستور کار خود قرار دهند تا از آلودگی منابع آب به فاضلاب­های شهری، صنعتی و کشاورزی یا شیرابه زباله جلوگیری یا به حداقل برسد تا از صرف هزینه­های هنگفت برای تصفیه و سالم سازی آب خودداری شود. به قول سعدی: « علاج واقعه پیش از وقوع باید کرد دریغ سود ندارد چو رفت کار از دست به روزگار سلامت سلاح جنگ بساز وگرنه سیل چو بگرفت، سد نشاید بست»
