fa اندازه‌گیری فشار داخل جمجمه در بیماران دچار صدمات تروماتیک مغزی در بیمارستان باهنر کرمان: یک گزارش کوتاه Intracranial Pressure Monitoring in Patients with Traumatic Head Injuries in Kerman Bahonar Hospital: A Short Report جراحي مغز و اعصاب جراحي مغز و اعصاب پژوهشي Research <div style="text-align: right "> gte vml 1]><v:roundrect id="_x0000_s1026" style='position:absoluteleft:0text-align:left margin-left:5.45ptmargin-top:-.1ptwidth:449.5ptheight:367.4ptz-index:-1' arcsize="1475f"> <w:wrap anchorx="page"/> </v:roundrect><img alt="AWT IMAGE" width="602" height="492" src="file:///C:/DOCUME~1/ADMINI~1/LOCALS~1/Temp/msohtmlclip1/01/clip_image001.gif" v:shapes="_x0000_s1026" >چکیده<o:p /> زمینه و هدف: هر چند اندازه‌گیری فشار داخل جمجمه جزء درمان استاندارد آسیب‌های تروماتیک مغزی است ولی هنوز از آن به صورت متداول در ایران استفاده نمی‌‌شود. هدف از این مطالعه اندازه‌گیری فشار داخل جمجمه در بیماران دچار صدمات تروماتیک مغزی در بیمارستان باهنر کرمان می‌‌باشد.<o:p /> مواد و روش‌ها: در این مطالعه مقطعی طی یکسال تمام بیمارانی که به علت آسیب تروماتیک مغزی به این مرکز مراجعه کرده بودند و تحت اندازه‌گیری فشار داخل جمجمه قرار گرفته بودند بررسی شدند. یافته‌های بالینی و رادیولوژیک زمان بستری و مرگ و میر بیماران و عوارض مرتبط با کاتتر اندازه‌گیری فشار داخل جمجمه مورد بررسی قرار گرفتند. پیامد ۶ ماهه بیماران طبق نمره Glascow Outcome Scale (GOS)ارزیابی شد. برای بررسی تأثیر عوامل بالینی بر پیامد بیماران از آزمون t مستقل برای متغیرهای کمی و آزمون مجذور کای برای متغیرهای کیفی استفاده شد.<o:p /> یافته‌ها: در این مدت ۱٨ بیمار ضربه مغزی تحت اندازه‌گیری فشار داخل جمجمه قرار گرفتند. هشت بیمارتحت درمان با کومای باربیتورات قرار گرفتند و ٨ عمل جراحی روی ٦ بیمار بر اساس فشار داخل جمجمه انجام شد. هر سه بیماری که فشار داخل جمجمه در آنها قابل کنترل نبود در حین بستری فوت شدند.GCS (نمره کما بر اساس معیار گلاسکو) بالا در هنگام بستری، فشار داخل جمجمه قابل کنترل و واکنش مردمک به نور با GOS بهتری در ٦ ماه بعد از تروما همراه بود.<o:p /> نتیجه‌گیری: به ‌نظر می‌رسد انجام مانیتورینگ فشار داخل جمجمه یک اقدام مفید و کم خطر در درمان بیماران ضربه مغزی باشد ولی لازم است مطالعاتی با گروه شاهد و حجم نمونه بیشتر انجام شود.<o:p /> واژه‌های کلیدی: آسیب تروماتیک مغز، پیش آگهی، فشار داخل جمجمه<o:p /></div> Background and Objective:<span style="font-family: 'Times New Roman', serif background-image: initial background-attachment: initial background-origin: initial background-clip: initial background-color: white background-position: initial initial background-repeat: initial initial "> Although intracranial pressure (ICP) monitoring is considered as the standard care for traumatic brain injuries, it is not routinely used in Iran. Hence, the purpose of this study is to reveal the primary results of the applying ICP monitoring in patients with traumatic head injuries in Bahonar Hospital in Kerman.<o:p /> Materials and Methods:The present study is a cross-sectional one in which all the patients with traumatic brain injuries referred to this given center and received the ICP monitoring were evaluated. We examined the clinical and imaging findings at admission as well as mortality and the complications related to the ICP catheter. The outcome was assessed according to the Glascow Outcome Scale (GOS) at six months after occuring trauma.The data were analyzed using t-test for quantitative variables and Chi-square test for categorical ones.<o:p /> Results: Eighteen patients with traumatic brain injury were undergone ICP monitoring during this time. Eight operations were performed on 6 patients based on their ICP. All the three patients with refractory high ICP died during hospital admission. Higher primary GCS and reactive pupil, were related to the better GOS scores six months after trauma.<o:p /> Conclusion: It seems that ICP monitoring is a safe and effective treatment modality in patients with traumatic brain injury.
