fa بررسی فراوانی مقاومت آنتیبیوتیکی در ایزوله‌های کلینیکی کلستریدیوئیدس دیفیسیل به آنتی‌بیوتیک‌های رایج درمانی در بیماران بستری در بیمارستان‌های آموزشی شهر کرمان طی سال‌های ۱۳۹۷-۱۳۹۹ The Frequency of Antibiotic Resistance of Clinical Isolates of Clostridioides difficile to Commonly Used Antibiotics in Hospitalized Patients of Educational Hospitals of Kerman City During 2018-2020 ميكروبيولوژي Microbiology پژوهشي Research زمینه و هدف: کلستریدیوئیدس دیفیسیل یکی از عوامل اصلی اسهال مرتبط با آنتی‌بیوتیک میباشد. پی بردن به الگوی مقاومت آنتیبیوتیکی آن به منظور کاهش شیوع و همچنین درمان عفونت ناشی از این باکتری (Clostridioides difficile infection; CDI) از اهمیت بهسزایی برخوردار است. هدف از انجام این تحقیق تعیین مقاومت آنتیبیوتیکی کلستریدیوئیدس دیفیسیل نسبت به آنتی‌بیوتیک‌های رایج درمانی، خصوصاً ونکومایسین و مترونیدازول به‌عنوان مؤثرترین راه درمان عفونت کلستریدیوئیدس دیفیسیل میباشد. مواد و روش ها: در این مطالعه توصیفی، ۴۱۷ نمونه مدفوع اسهالی از بیماران بستری در بیمارستانهای آموزشی شهر کرمان طی سالهای ۱۳۹۷ تا ۱۳۹۹ گرفته شد. نمونهها روی محیط کشت سیکلوسرین سفوکسیتین فروکتوز آگار کشت و ایزولههای مشکوک به کلستریدیوئیدس دیفیسیل جداسازی شدند. شناسایی ژن cdd-3 برای تشخیص قطعی باکتری صورت پذیرفت. مقاومت آنتی‌بیوتیکی به روش انتشار دیسک و با استفاده از دیسک‌های ونکومایسین، مترونیدازول، ریفامپین، آموکسی‌سیلین/کلاولانیک اسید، اریترومایسین، ایمیپنم، سیپروفلوکساسین و کلیندامایسین بررسی شد. نتایج به صورت تعداد و درصد گزارش شد. یافته ها: در مجموع ۶۸ (۳/۱۶ درصد) ایزوله کلستریدیوئیدس دیفیسیل جداسازی شد. اکثر ایزولهها به ونکومایسین و مترونیدازول حساس بودند درحالیکه میزان مقاومت این ایزولهها به سیپروفلوکساسین و کلیندامایسین بالا بود. فراوانی ایزولههای مقاوم به چند دارو ۹/۷۷ درصد مشاهده شد. نتیجهگیری: نتایج این مطالعه نشان داد که ونکومایسین هنوز داروی انتخابی مناسب برای درمان عفونت کلستریدیوئیدس دیفیسیل میباشد. همچنین، فراوانی ایزولههای مقاوم به آنتیبیوتیکهای القا کننده عفونت کلستریدیوئیدس دیفیسیل (اریترومایسین، کلینداماسین، سیپروفلوکساسین) و ایزولههای مقاوم به چند دارو زیاد میباشد. لذا با تجویز مناسب آنتی بیوتیک میتوان از گسترش سویه های مقاوم به آنتیبیوتیک این باکتری جلوگیری کرد. واژه های کلیدی: کلستریدیوئیدس دیفیسیل، مقاومت آنتیبیوتیکی، مقاومت چند دارویی، کرمان Received: 18/09/22 Sent for Revision: 23/10/22 Received Revised Manuscript: 13/12/22 Accepted: 14/12/22 Background and Objectives: Clostridioides difficile (C. difficile) is one of the main causes of antibiotic-associated diarrhea. It is important to find out the pattern of its antimicrobial susceptibility for reducing prevalence and also treating C. difficile infection (CDI). This study aims to investigate the antimicrobial resistance of C. difficile to commonly used antibiotics, especially vancomycin and metronidazole, as the most effective antibiotics against the bacterium. Materials and Methods: In this descriptive study, 417 diarrheal stool samples were taken from hospitalized patients of educational hospitals of Kerman City from 2018 to 2020. The samples were cultured on cycloserine-cefoxitin fructose agar (CCFA), and suspected C. difficile colonies were isolated. Identification of the cdd-3 gene for definitive diagnosis of C. difficile was performed. Antibiotic resistance test was conducted by the disk-diffusion method using vancomycin, metronidazole, rifampin, amoxicillin-clavulanic acid, erythromycin, imipenem, ciprofloxacin, and clindamycin disks. The results were reported as numbers and percentages. Results: A total of 68 (16.3%) isolates of C. difficile were taken from the samples. Most C. difficile strains were susceptible to vancomycin and metronidazole, while the highest rate of resistance was related to ciprofloxacin and clindamycin. The prevalence of multi-drug resistant (MDR) strains was 77.9%. Conclusion: The results of this study showed that vancomycin is still the best antibiotic for treating CDI. Also, the frequency of the isolates resistant to CDI-inducing antibiotics (erythromycin, clindamycin, ciprofloxacin) and MDR isolates was high. Therefore, the spread of resistant strains of C. difficile can be prevented by appropriate antibiotic prescription.
