Background and Objectives: Relationship Obsessive-Compulsive Disorder (ROCD) is a relatively new subtype of obsessive-compulsive disorder characterized by symptoms focused on romantic relationships. The present study aimed to examine the structural relationship between ROCD symptoms and marital satisfaction, considering the mediating role of jealousy.
Materials and Methods: This descriptive-correlational study employed Structural Equation Modeling (SEM) for data analysis. The statistical population included all married individuals residing in Mashhad in 2023, from which 350 participants were selected through convenience sampling. Research instruments comprised the Hendrick Marital Quality Scale (1988), the Relationship Obsessive-Compulsive Symptoms Questionnaire (Doron et al., 2012), and the Multidimensional Jealousy Scale (Pfeiffer & Wong, 1989). Data were analyzed using Pearson’s correlation and SEM.
Results: The mean and standard deviation scores were 20.92±15.17 for relationship obsessive-compulsive symptoms, 68.73±42.64 for jealousy, and 23.04±7.07 for marital satisfaction. In addition, findings indicated a significant positive correlation between ROCD symptoms and jealousy (r=0.49, p<0.001), and a significant negative correlation between ROCD symptoms and marital satisfaction
 (r=-0.54, p<0.001). Additionally, jealousy was negatively and significantly associated with marital satisfaction (r=-0.69, p<0.001). Structural equation modeling revealed that jealousy serves as a significant mediator in the relationship between ROCD symptoms and marital satisfaction (r=-0.27, p<0.001).

Conclusion: The present study showed that symptoms of relational obsessive-compulsive disorder (ROCD) affect marital satisfaction through the mediating role of jealousy; a finding that emphasizes the need to design therapeutic interventions focused on reducing jealousy and modifying cognitive-emotional patterns related to ROCD in marital satisfaction.
